From Manual Work to Major Gifts – How Hoag Transformed Its Grateful Patient Program

What happens when a grateful patient program stops running on spreadsheets, manual uploads, and disconnected systems and starts running in real time? In this session, Hoag Hospital Foundation will share how it transformed a complex, labor-intensive process into a more connected and scalable fundraising engine, cutting weekly processing time from 13 hours to 10–15 minutes while building a stronger major gift pipeline.

Attendees will hear how Hoag integrated Epic data into Ascend, automated screening and prospect workflows, improved physician-development coordination, and created a faster path from patient identification to meaningful donor outreach. The result was not just efficiency, but measurable fundraising impact: more than $5.1 million raised in the first half of 2026, 12 active major gift proposals, and 50,000 potential prospects identified for future engagement.
