Through the generosity of Holmes, Radford & Avalon, a full service consulting firm based in St. Louis, Missouri, a scholarship has been established for Association for Healthcare Philanthropy members within the Midwest Region to attend the annual 2008 AHP Midwest Regional Conference. Holmes, Radford & Avalon will grant a $395 scholarship award to fund the conference registration fees for one conference attendee. The 2008 AHP Midwest Regional Conference will be held April 27-29, 2008 at the Hyatt Regency Louisville – Louisville, KY. The recipient’s organization will be reimbursed $395 for conference registration fees. Upon receipt of a written summary of an awardees’ experience, the scholarship funds will be disbursed. Scholarship applications must be submitted no later than Thursday, January 31, 2008. The Scholarship Committee will review all applications and base their recommendations on expressed financial need, clear objective for use of the scholarship and goals for career development as well as any additional notes on the applicant’s form. The scholarship committee will announce its selection of the recipient by Friday, February 29, 2008. All applicants will be notified of the committee selection and decisions will be final. SELECTION CRITERIA 1) The applicant must be a current member of the Association for Healthcare Philanthropy in the Midwest region working in a healthcare institution at the time of application. 2) The applicant must have no more than five years experience in health care philanthropy. 3) The applicant must submit the completed scholarship application form along with a written statement including a brief history of his/her development experience; the reasons he/she wishes to attend the AHP Midwest Regional Conference; and the reason he/she is deserving of the scholarship award. 4) The application and written statement must be submitted simultaneously and postmarked no later than Thursday, January 31, 2008. CONDITIONS OF AWARD 1) The scholarship award must be used to offset the conference registration fees at the 2008 AHP Midwest Regional Conference. 2) The funds will be applied toward conference registration fees only. The recipient is responsible for paying all registration fees in advance. The scholarship award will be disbursed by Holmes, Radford & Avalon upon receipt of a written summary of the recipient’s educational experience. This written summary must be submitted by May 31, 2008. 3) The recipient is responsible for all costs beyond the conference registration fees, including transportation, housing and per diem expenses to attend the Conference.
